Activity Overview

Attendees of this session will be able to recognize social determinants of health and to provide guidance on a number of concrete actions that they can take to address social determinants of health in their community pharmacies to improve patient health and reduce inequities. The session will review how social determinants are linked to health outcomes and how healthcare providers can address social needs of patients through awareness, adjustment, assistance, alignment, and advocacy. In addition, this session will outline steps that pharmacists can take to support patients at the patient level (including how to ask patients about social challenges in asensitive and caring way), at the practice level (including providing examples of targeted interventions in different practice settings), and at the community level. Furthermore, clinical decision aids, practice guidelines and other tools will be shared to help pharmacists and other healthcare professionals address social determinants of health in their day-to-day practice.

The American Society of Health-System Pharmacists is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education.

All pharmacists with roles in community pharmacy and ambulatory care pharmacy practices.

After participating in this CPE activity, participants should be able to:

  • Describe the impact of social determinants of health on people's health, well-being, and quality of life.
  • Identify ways that community pharmacists can respond to social needs of diverse patient populations at the patient, practice, and community levels.
  • Given a scenario, develop an action plan that can have positive impacts on a patient's social challenges, wellness, quality of life, and health outcomes.
