Activity Overview

Polypharmacy is increasingly common in patients living with chronic kidney disease (CKD), yet its risks often go unrecognized in clinical practice. This webinar is designed to equip healthcare professionals with the knowledge and practical skills needed to identify medication-related harms, reduce inappropriate medication use, and improve patient outcomes through safe and effective deprescribing.

Through realworld patient case scenarios, participants will explore how complex medication regimens can negatively affect safety, kidney function, and overall quality of life. The session will highlight common medication therapy problems seen in chronic disease management, particularly when kidney impairment complicates medication selection, dosing, and monitoring.

Learners will also be guided stepbystep through an evidenceinformed deprescribing framework. By breaking down the five key components of the deprescribing process, the webinar will empower clinicians to develop individualized action plans they can apply in their own practice settings.

Join us to strengthen your confidence in managing complex medication regimens and to enhance patient-centered care for individuals with chronic kidney disease.
