Activity Overview
Digital health technologies continue to reshape pharmacy practice by enabling pharmacists and care teams to access comprehensive patient information, provide timely interventions, and expand access to care through virtual services. This webinar will highlight real-world case studies demonstrating how organizations are implementing innovative digital health models, including on-demand telehealth services, remote patient monitoring, and maximizingthe use of patient-generated health data to improve patient outcomes. Speakers will describe the catalysts that prompted development, the implementation process, and impact of these tools. The session will share strategies for integrating digital health innovations to enhance patient engagement and care delivery.
